❖ Lighting rebate(s) must be submitted no later than 3 months after the purchase date and must be installed on the cooperative’s lines. 

❖ Rebates are in place through December 31, 2026 or until funds are depleted. Members are encouraged to submit as soon as possible.  

❖ Rebate not to exceed 50% of the cost of equipment. Additional eligibility criteria may apply. Contact the cooperative for details. 

❖ Rebates submitted after December 31, 2026, will not qualify if the rebate is no longer offered in 2027. 

❖ Bulbs receiving promotional pricing from Freeborn Mower Electric Cooperative at Ace Hardware, Dollar Tree (Albert Lea), Home Depot, and Walmart do not qualify for the rebate. 

❖ All required documentation listed below is required and must be submitted no later than 3 months after the purchase date.

✓ Submit this rebate form. 

✓ A dated copy of your receipt or invoice for each item purchased. 

✓ For LED bulbs & fixtures, include UPC Code and Lighting Facts from the package. 

✓ For all fixtures, include packaging or documentation showing the number of lumens or number of bulbs per fixture as required.
